Changes in the conditions of health insurance for children of foreigners residing in the Czech Republic


As of January 1, 2024, as a result of the amendment to the Act on the Residence of Foreigners, the conditions for child insurance in the Czech Republic have changed. Newly, public health insurance is compulsory for:


1) all foreigners under the age of 18 who have a valid long-term residence permit,

2) children or minors born in the territory of the Czech Republic to a mother with a long-term residence permit in the territory of the Czech Republic, for whom an application for a long-term residence permit was submitted within a period of 60 days.


The responsibility and reporting obligation for registering a child/minor for public health insurance rests with his/her legal representative.



1. Foreigners under the age of 18 with a valid long-term residence permit


The obligation to register for public health insurance depends on whether the child has a long-term residence permit and whether commercial health insurance for foreigners has been arranged as of January 1, 2024:


In case that a) the child has a long-term residence permit and has commercial health insurance for foreigners as of January 1, 2024, he/she becomes enrolled in the public health insurance the day after the end of commercial health insurance for foreigners. A child must be registered with the selected insurance company no later than 8 calendar days before the commercial insurance expires.

In case that b) the child receives a long-term residence permit only after January 1, 2024, enrolment in the public health insurance is automatically established by receiving a long-term residence permit. In this case, the child must be registered with the selected insurance company within 8 calendar days after receiving the permit.



2. A child born in the territory of the Czech Republic to a mother who has been issued a valid permit for long-term residence in the territory of the Czech Republic


The birth of a child whose mother holds a valid long-term residence permit in the Czech Republic must be reported within 8 days from the day of his/her birth to the health insurance company with which the child's mother is insured. If the mother does not have public medical insurance, then it is necessary to report it to the father’s insurance company.

As of 1 January 2024, the child is enrolled in public health insurance for the entire duration of the procedure for the issuance of a long-term residence permit, provided that it was submitted within 60 days from the day of his/her birth. How to register your child?


When registering the child at the branch of the selected insurance company, the legal representative submits:


• residence permit of a child, or Confirmation of submission of the application

• birth certificate of the child,

• legal representative's passport,

• a document proving the termination of commercial insurance (only for the situation described above in point 1a),

• residence permit of the mother who has a long-term residence permit in the Czech Republic in the case of a child born in the Czech Republic to a mother with a long-term residence permit, for whom an application for a long-term residence permit was submitted within 60 days (only for the situation described above in point 2).

• a request for approval of long-term/permanent residence in the event that a child born in the territory has to submit a residence permit that has not yet been approved by the Ministry of Interior at the time of application (only in the case of the situation described above in point 2).


In case that one of the child's parents holds a valid permanent residence permit, the child's health insurance is fully covered by the state, if the application for a permanent residence permit for him/her was submitted within 60 days of his birth. When registering the child at the branch of the selected insurance company, the parent submits the confirmation of the application.


If neither parent holds a permanent residence permit, or the mother does not hold a long-term residence permit, the child must be insured on the basis of comprehensive health insurance from birth with an insurance company that is authorized to provide this insurance in the territory of the Czech Republic.


Validity of public health insurance

A child will be a enrolled in the public health insurance system only until reaching the age of 18 or until the end of the validity of the long-term residence. After reaching the age of 18, it is necessary to take out commercial health insurance. In this case, it is recommended to contact one of the insurance companies that offer commercial health insurance products for foreigners.


Health insurance payments

The payer of the insurance premium for a minor foreigner is his/her legal representative. The monthly insurance premium is 13.5% of the minimum wage for the given year and is thus the same as in the case of persons without taxable income (OBZP). As of January 1, 2024, the amount of the OBZP insurance premium is CZK 2,552.
